People die all the time from State-provided reasons, too.
Why is it that the per capita murder rates are higher in Washington DC, New York City, San Francisco, Los Angeles, Boston and Chicago, where you have to be politically-connected to own and carry a weapon legally than in Vermont, Alaska and Arizona, where you just strap your pistol on your belt and go about your business?
How about the FDA holding up approval on beta blockers, which were used in Europe in the 1970s? The FDA finally got around to approving them in 1981. What about the 100,000 who possibly died from heart attacks while waiting for that approval? Who speaks for them? See A Killer Agency by Dr. Walter E. Williams.
I and other Libertarians aren’t advocating a world without rules at all. Last time I checked, gravity is a rule, and it’s pretty much unrepealable at this time. The laws of supply and demand are likewise rules. The Non-Aggression Principle that forms the core of Libertarian thought is also a rule.
Rules of the statutory or regulatory kind that are incompatible with the Non-Aggression Principle are what Libertarians oppose.
